Your Day.


As a Population Health Project Manager you need to know how to:
 

  • Run, manipulate, and display Crystal Report data
  • Complete all work in compliance with HIPAA and Primary Health Medical Group Policies and Procedures
  • Maintain the status of value-based projects
  • Participate in value-based project meetings and work with insurance companies and networks
  • Coordinate and organize key operational processes related to all projects assigned
  • Assist with the development and maintenance of processes and tools for patients and staff
  • Maintain patient registries around attribution, chronic conditions, and patient outreach
  • Work with internal and external entities as needed to exchange data, as well as with records vendors and facilities to coordinate patient records
  • Communicate with patients in relation to attribution issues
  • Work closely with staff at all levels
  • Perform other projects and duties as assigned by the Director of Population Health


Your experience.

  • High school diploma or equivalent; College degree preferred
  • Three or more years of administrative work in a medical or medical insurance field
  • Extensive knowledge of eCW (eClinicalWorks) is required
  • Medical clinic operations experience preferred
  • Word processing and spreadsheet knowledge such as Excel
  • Knowledge of population health terms, quality metrics, and data points
  • Ability to use internal and external Business Intelligence tools
  • Outstanding communication skills, both written and verbal 
  • Strong organizational skills
  • Ability to multitask and work professionally in stressful circumstances
  • Excellent internal and external customer service required with both patients and employees
